OBJECTIVES: We aimed to translate and linguistically and cross-culturally validate Sheffield Profile for Assessment and Referral for Care (SPARC) in Spanish for Colombia (SPARC-Sp). METHODS: The linguistic validation of SPARC followed a standard methodology. We conducted focus groups to assess the comprehensibility and feasibility. The acceptability was assessed using a survey study with potential users. RESULTS: The comprehensibility assessment showed that additional adjustments to those made during the translation-back-translation process were required to apply SPARC-Sp in rural and low-schooled populations. It also identified the need for alternative administration mechanisms for illiterate people. The acceptability survey showed that potential users found SPARC-Sp as not only acceptable but also highly desirable. However, they desired to expand the number of items in all domains. SIGNIFICANCE OF RESULTS: Beyond the semantic and conceptual validity attained through the back-translation process, actual cultural validity could be acquired thanks to the comprehensibility tests. Although extending the instrument is something potential users would like to do, it would make it less feasible to utilize the SPARC-Sp in clinical settings. Nonetheless, the instrument might benefit from the inclusion of a domain that evaluates challenges encountered when accessing the health-care system. For communities lacking literacy, alternate administration methods must also be considered.

Introducción: la fractura de cadera en adultos mayores constituye un problema de salud importante, ya que está asociada a mayor morbilidad y mortalidad. Por esta razón, se han creado protocolos de ortogeriatría para su abordaje, los cuales reportan resultados favorables.Objetivo: evaluar los desenlaces adversos perioperatorios en adultos mayores con fractura de cadera antes y después de la implementación de un protocolo de evaluación geriátrica integral (EGI).Metodología: estudio de casos y controles realizado en 136 pacientes (casos: 43; controles: 93) >65 años con fractura de cadera que fueron atendidos en un hospital de referencia entre 2020 y 2021 (casos), y 2015 y 2017 (controles). Se determinaron diferencias entre grupos mediante un análisis bivariado usando las pruebas de chi cuadrado o exacta de Fisher en variables categóricas, y las pruebas t de student o de Wilcoxon de los rangos con signo en variables continuas según la distribución de los datos.Resultados: los desenlaces adversos perioperatorios ocurrieron en el 62,7% de los casos y el 84,0% de los controles (p=0,007; OR:0,32; IC95%:0,13-0,73). La mediana de duración de la estancia hospitalaria fue 10 y 17 días en los casos y controles, respectivamente (RIQ: 8-14; RIQ 11-26), mientras que el tiempo entre el ingreso a urgencias y la cirugía fue 7 días (RIQ:5-11) y 11 días (RIQ:8-17), respectivamente (p=0.002). Conclusiones: la implementación de la EGI redujo los desenlaces adversos por fractura de cadera, principal-mente la duración de la estancia hospitalaria y el tiempo entre el ingreso y la cirugía, donde se observaron diferencias estadísticamente significativas entre ambos grupos


Introduction: Hip fracture in elderly patients is a major health concern, as it is associated with increased morbidity and mortality. For this reason, orthogeriatric protocols have been created to approach it, reporting favorable outcomes.Objective: To evaluate adverse perioperative outcomes in elderly patients with hip fractures before and after the implementation of a comprehensive geriatric assessment (CGA) protocol.Methodology: Case-control study performed on 136 patients (cases: 43; controls: 93) >65 years old with a hip fracture, who were treated at a referral hospital between 2020 and 2021 (cases) and 2015 and 2017 (controls). Differences between groups were determined through bivariate analysis using Fisher's chi-square or exact tests on categorical variables, and Student's t-test or Wilcoxon signed-rank test on continuous variables depending on the distribution of the data.Results: Adverse perioperative outcomes occurred in 62.7% of cases and 84.0% of controls (p=0.007; OR:0.32; 95%CI:0.13-0.73). The median length of hospital stay was 10 and 17 days in cases and controls, respectively (IQR: 8-14; IQR 11-26), while the time between ED admission and surgery was 7 days (IQR:5-11) and 11 days (IQR:8-17), respectively (p=0.002).Conclusions: The implementation of the CGA protocol reduced adverse outcomes associated with hip fracture, mainly the length of hospital stay and the time between admission and surgery, in which statistically significant differences were observed between both groups

Resumen La leptospirosis en una enfermedad infecciosa importante en el diagnóstico diferencial de un paciente con síndrome ictérico hemorrágico, especialmente en países de ubicación tropical como Colombia. A continuación, se presenta el caso de un paciente masculino de 21 años, con cuadro clínico agudo caracterizado por presencia de fiebre y elevación de bilirrubinas, asociado a diarrea y trombocitopenia. Inicialmente se hizo diagnóstico presuntivo de dengue, sin embargo, durante su evolución presentó hemorragia y dificultad respiratoria asociada a secreción hemática con necesidad de intubación endotraqueal, por lo que se solicitó serologia IgM seriada de leptospira pensando en esta entidad como diagnóstico diferencial luego de indagar antecedentes epidemiológicos, la cual tuvo reporte positivo. Es importante conocer este tipo de casos debido a la presentación de compromiso pulmonar como una manifestación rara pero altamente asociada a mortalidad. Finalmente, con manejo de soporte y antibiótico adecuado se obtuvo una buena evolución clínica de este paciente.


Abstract Leptospirosis is a relevant infectious disease when there is a patient with an ictero-hemorrhagic syndrome, especially in tropical countries such as Colombia. The following is the case of a 21-year-old male patient, with an acute clinical picture given by fever, elevated bilirubin, associated with diarrhea and thrombocytopenia. Initially, a presumptive diagnosis of dengue was made. However, during his evolution, he presented hemorrhage and respiratory distress with a hematic secretion that required endotracheal intubation. After inquiring about the epidemiological data, leptospirosis was considered a differential diagnosis. Serial serology for leptospira IgM was requested and was positive. It is essential to know this type of case due to pulmonary affections as a rare manifestation but highly associated with mortality. Finally, with adequate antibiotic and supportive management, a good clinical evolution of this patient was obtained.

Resumen La hiponatremia es el trastorno hidroelectrolítico más frecuente observado en pacientes hospitalizados y es importante resaltar que se ha asociado a morbilidad y mortalidad de estos. Esta entidad representa un proceso fisiopatológico relacionado con una alteración en la homeostasis del agua, en la cual los pacientes presentan síntomas en su mayoría neurológicos, que se correlacionan con el nivel de sodio y el tiempo de aparición del trastorno. Se realizó una búsqueda en las bases de datos PubMed y Lilacs de monografías, artículos de revisión y artículos originales con el objetivo de revisar aspectos sobre la clínica, diagnóstico y manejo de la entidad. Para el diagnóstico, la batería de estudios se solicita en función del contexto clínico, actuando temprano y permitiendo la corrección del trastorno de acuerdo al escenario respectivo. La hiponatremia es un problema médico frecuente que bajo un abordaje práctico y sencillo permite tomar decisiones clínicas de forma oportuna. MÉD.UIS. 2020;33(2):85-93.


Abstract Hyponatremia is the most frequent hydroelectrolytic disorder observed in hospitalized patients and it is important to note that it has been associated with morbidity and mortality in this patients. This entity represents a pathophysiological process related to an alteration in water homeostasis, in which patients present symptoms mostly neurological, that correlate with the sodium level and the time of onset of the disorder. A search of the PubMed and Lilacs databases of monographies, review articles and originals articles was performed with the objective to review clinical, diagnostic and management aspects of this entity. For diagnosis, the clinical laboratory studies are requested depending on the clinical context, acting early and allowing correction of the disorder according to the respective scenario. Hyponatremia is a frequent medical problem that requires a practical and simple approach favoring clinical decisions in a timely manner. MÉD.UIS. 2020;33(2):85-93.

INTRODUCTION: Pneumonic aspiration of gastric contents is an important complication of anesthetic practice. : OBJECTIVE: To determine the applicability of ultrasound for the assessment of the qualitative and quantitative characteristics of gastric contents. MATERIAL AND METHODS: A systematic review was carried out, based on a structured search of the literature published in the databases: Pubmed, EMBASE, SciELO, Cochrane Library, OVID and Google Scholar, from the foundation of these until December 2018 Boolean connectors were used to locate information from the terms: anesthesiology, ultrasonography, gastrointestinal Contents. The quality of the information was assessed using different tools according to each type of study. RESULTS: We found 23 relevant studies for inclusion in the systematic review. The overall risk of bias was low. CONCLUSIONS: Gastric ultrasound seems to correspond to a tool that is easy to perform at the patient's bedside, but there is uncertainty about its applicability in uncontrolled settings, different from those applied in the studies described in this review. More research is needed with different populations and scenarios, in favor of establishing the behavior in these, of the variables that can be determined by ultrasound. The implications for clinical practice and long-term outcomes may have the measurements achieved by ultrasound are still uncertain.


INTRODUCCIÓN: La aspiración neumónica del contenido gástrico es una complicación importante de la práctica anestésica. OBJETIVO: Determinar la aplicabilidad del ultrasonido para la valoración de las características cualitativas y cuantitativas del contenido gástrico. MATERIALES Y MÉTODOS: Se realizó una revisión sistemática, a partir de una búsqueda estructurada de la literatura publicada en las bases de datos: Pubmed, EMBASE, SciELO, Cochrane Library, OVID y Google Scholar, desde la fundación de las mismas hasta diciembre de 2018. Se utilizaron conectores booleanos para localizar información a partir de los términos: anesthesiology, ultrasonography, gastrointestinal Contents. La calidad de la información fue valorada mediante diferentes herramientas de acuerdo a cada tipo de estudio. RESULTADOS: Se encontraron 23 estudios pertinentes para su inclusión en la revisión sistemática. El riesgo global de sesgo fue bajo. CONCLUSIONES: El ultrasonido gástrico parece corresponder a una herramienta de fácil realización a la cabecera del paciente, pero existe incertidumbre sobre su aplicabilidad en contextos no controlados, diferentes a los aplicados en los estudios descritos en la presente revisión. Se necesita más investigación con distintas poblaciones y escenarios, en favor de establecer el comportamiento en estos, de las variables que pueden determinarse por ultrasonido. Las implicaciones que para la práctica clínica y desenlaces a largo plazo puedan tener las mediciones logradas mediante ecografía son aún inciertas.

RESUMO

Objetivo: el objetivo de la presente investigación, es describir las actitudes frente a la prevención del cáncer de cuello uterino en mujeres del instituto Formarte, de la ciudad de Cali- Colombia. Materiales y métodos: se realizó un estudio de corte transversal. Se incluyeron 138 mujeres matriculadas en cualquiera de los programas académicos, a quienes se les realizó una encuesta con el fin de medir las variables propósito de la investigación. Resultados: se encontró una media de edad de 33,7 años (DE 13,2). De estas, 22,5% iniciaron relaciones por debajo de los 14 años, 42,8% informó haber tenido al menos 3 compañeros sexuales, 48% rara vez recurre al uso del condón, 28,3% refirió el consumo de cigarrillo. El 23,2% ha recibido algún tipo de educación respecto a la prevención del cáncer de cérvix, aunque el 82,6% reconoce la importancia de la citología y en su mayoría, 42% se realiza el examen anualmente. Conclusiones: se encontraron deficiencias respecto a actitudes en la prevención del cáncer de cuello uterino, las cuales parecen estar relacionadas con el contexto cultural particular de la población estudiada y que guardan disimilitud con las observadas en otras localizaciones geográficas..(AU)


Objective: the objective of the present investigation is to describe the attitudes towards the prevention of cervical cancer in women from Formarte Institute in the city of Cali- Colombia. Materials and methods: a cross-sectional study was carried out. We included 138 women enrolled in any of the academic programs, who were surveyed in order to measure the variables of the research. Results: an average age of 33,7 years was found (SD 13,2). Of these, 22,5% began relationships below 14 years of age, 42,8% reported having had at least 3 sexual partners, 48% rarely resorted to condom use,28,3% reported smoking. 23,2% have received some type of education regarding the prevention of cervical cancer, although 82,6% recognize the importance of cytology and in the majority, 42% perform the exam annually. Conclusions: We found deficiencies regarding attitudes in the prevention of cervical cancer, which seem to be related to the particular cultural context of the population studied and that are dissimilar to those observed in other geographical locations..(AU)

RESUMO

The Cry11 family belongs to a large group of δ-endotoxins that share three distinct structural domains. Among the dipteran-active toxins referred to as three-domain Cry11 toxins, the Cry11Aa protein from Bacillus thuringiensis subsp. israelensis (Bti) has been the most extensively studied. Despite the potential of Bti as an effective biological control agent, the understanding of Cry11 toxins remains incomplete. In this study, five Cry11 variants obtained via DNA shuffling displayed toxic activity against Aedes aegypti and Culex quinquefasciatus. Three of these Cry11 variants (8, 23, and 79) were characterized via 3D modeling and analysis of docking with ALP1. The relevant mutations in these variants, such as deletions, insertions and point mutations, are discussed in relation to their structural domains, toxic activities and toxin-receptor interactions. Importantly, deletion of the N-terminal segment in domain I was not associated with any change in toxic activity, and domain III exhibited higher sequence variability than domains I and II. Variant 8 exhibited up to 3.78- and 6.09-fold higher toxicity to A. aegypti than Cry11Bb and Cry11Aa, respectively. Importantly, variant 79 showed an α-helix conformation at the C-terminus and formed crystals retaining toxic activity. These findings indicate that five Cry11 variants were preferentially reassembled from the cry11Aa gene during DNA shuffling. The mutations described in loop 2 and loop 3 of domain II provide valuable information regarding the activity of Cry11 toxins against A. aegypti and C. quinquefasciatus larvae and reveal new insights into the application of directed evolution strategies to study the genetic variability of specific domains in cry11 family genes.
